Procedure
1
Within 21 days of occurrence
Fee: Nil- Institutions/hospitals apply online on behalf of the citizen.
- For non-institutional/home death, family member(s) apply offline to the respective Borough Office.

After 21 days, within 30 days
Fee: Rs. 1- Institutions/hospitals and home death cases - family member(s) apply offline to the respective Borough Office.

After 30 days, within 1 year
Fee: Rs. 2- Institutions/hospitals and home death cases - family member(s) apply to the respective Borough Office.

Delayed registration (after 1 year)
Fee: Rs. 10- Apply online to the concerned SDO through the e-district portal of GoWB for the necessary order for issuance of the certificate, then apply offline at the Corporation Head Quarter.
Documents required (first three cases)
- Photocopy of I.D. proof of applicant (family member)
- Photocopy of I.D. proof of the deceased person
- Death certificate issued by the concerned institution/hospital
- In case of non-institutional/home death, death certificate given by the attending doctor
- Cremation / burial certificate
- Notarial affidavit (only for cases beyond 30 days)
Documents required (delayed registration)
- Cremation certificate
- Death certificate from institution or attending doctor
- ID proof of applicant
- ID proof of the deceased person
- Residential certificate by local Councillor of BMC / Executive Officer of respective Municipality / Pradhan of concerned Gram Panchayat
- Declaration by 5 local residents with self-attested photocopies of their Aadhaar card / voter ID
- All documents are to be submitted through the online e-district portal of Govt. of West Bengal
